Through this service, you can request the amendment of a social care facility license or a social care facility branch license in the Emirate of Abu Dhabi
- Service terms and conditions: Any social care facility or branch licensed in Abu Dhabi can apply for this service.
- Required Documents:
- Document explaining the required amendment, including an overview of the program or service, in addition to a list of the names of employees and beneficiaries "in case of modification to programs and services"
- Copy of the academic qualifications and experience certificates of the technical and administrative staff "in case of adding a service or program"
- Valid copy of the Emirates ID from both sides for partners "in case of amending partners"
- Valid copy of the main page of the passport of the partners "in case of modification of partners"
- Certificate of good conduct – criminal status examination "in case of amendment of partners"
- Copy of the academic qualifications of the partners "in case of modification of partners"
- Disclaimer undertaking form "in case of modification of partners"
- Applicant's CV "only for family counselling centre in case of modification of partners"
- Service Steps:
- Submit the application and the required documents through the available channels
- Receive DCD update to submit additional information or supported document if needed
- Receive final decision on request to amend social care facility/facility branch license through the available channel
