Through this service, you can request to modify the data of your social care professional license that is issued by Department of Community Development in Abu Dhabi
- Service terms and conditions: The social care professional must be licensed by DCD& has a valid license
- Required Documents:
- If the amendment request was on personal information, applicant should submit related support document such as valid passport/EID
- If the amendment request was on the professional license title, applicant should submit related support educational qualification + experience letter + DCD form for good standing
- If the amendment request was on the facility name, applicant should submit related support experience letter + DCD form for good standing
- If the amendment request is to update the personal photo, applicant should submit a passport sized colored portrait photo
- Service Steps:
- Submit the application along with the required documents through the available channels
- If the amendment request was on personal information, applicant should submit related support document such as valid passport
- If the amendment request was on the professional license title, applicant should submit related support educational qualification + experience letter
- If the amendment request was on the facility name, applicant should submit related support experience letter + DCD form
- Receive DCD update to submit additional document if needed
- Receive Preliminary approval on the request electronically, in addition to notification to fulfill required procedures for license issuance such as providing primary source verification report or passing professional competency assessment
- In the event of completing all the required procedures as per the notification, the application of amend social care professional license request will be completed & the amended license will be received automatically through the available channels
